Statement of Facts for establishing Live-born status
The following is a sample of a Statement of Facts for establishing yourself as a live-born human, not a corporate entity. Four copies of this affidavit should be (preferably) handwritten; one copy forwarded to the corporate entity which filed a complaint against you, and one copy sent to the court. Before entering the court, present a copy to the clerk for filing and have her stamp the other one. Keep that last copy with you in court.
You may also file this as a PUBLIC NOTICE in your local newspaper for at least four consecutive weeks to establish the separation of your flesh and blood body from the corporate entity. Have the newspaper provide a notarized affidavit that this Public Notice did run for the weeks specified and file a copy with your County Clerk, and keep one (notarized) for your own records.

I John Henry Doe, a living, breathing man, declare in my own handwriting that the following facts are true to the best of my knowledge and belief. I, John Henry Doe, admit and deny as follows:
ADMISSION
I admit that:
1. I was born alive on December 5, 1962 at Any Town, Maine to Jack and Sally Doe.
2. My proper given Christian name is John Henry Doe.
3. I am a live flesh and blood human being.
DENIAL
I deny as follows:
1. I deny that I am the non-juristic legal entity JOHN H. DOE; DOE, JOHN H. or any other artificially created all capital letter or Upper/Lower case version of a name that is similar to my Christian name.
2. I deny that I am either fiduciary or surety for said non-existent juristic entity JOHN H. DOE; DOE, JOHN H. or any other artificially created all capital letter or Upper/Lower case version of a name that is similar to my Christian name.
3. I hereby deny that the following corporations exist:

CONCLUSION
If any man or woman desires to answer this 'affidavit,' please answer in the manner of this affidavit, with notarized affidavit, using your Christian or family name for signature, and mail to the below named notary, address provided, within five (5) days or default will be obtained.
On the 25th day of June, 2000 A.D. a man who identified himself as John Henry Doe appeared before me, a notary, and attested to the truth of this affidavit with his signature.
